Privacy policy.
1. About This Policy
Found Voice Therapy, LLC ("Found Voice Therapy," "we," "us," or "our") operates the website located at foundvoicetherapy.com (the "Site"). This Privacy Policy explains what information we collect when you visit or use the Site, how that information is used and shared, and what choices you have.
This policy applies only to information collected through the Site. It does not govern protected health information created or maintained in the course of clinical services. That information is protected under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and applicable Mississippi and Alabama law, and its handling is described in our Notice of Privacy Practices, which is provided to clients at the beginning of services and is available on request.
By using the Site, you agree to the practices described in this policy. If you do not agree, please do not use the Site.
2. Information You Provide to Us
We collect the following information only when you choose to give it to us.
Contact form
If you submit the contact form, we collect the name, email address, and telephone number you enter, along with anything you type into the message field. The form is intended for general inquiries and requests to schedule a consultation. Please do not include detailed clinical, medical, or other sensitive personal information in the message field. See Section 8 regarding the security limits of web forms and email.
Email and telephone
If you contact us by email at contact@foundvoicetherapy.com or by telephone, we receive whatever information you choose to share in that communication.
Scheduling and the client portal
Appointment scheduling and the client portal are handled through Headway, a third-party platform. Information you enter there is collected by Headway under its own privacy policy and terms, and is not collected by this Site. We encourage you to review Headway’s privacy policy before creating an account.
Clinical records
Once you become a client, your clinical record is maintained in Headway, our electronic health record system, which operates as a HIPAA business associate. Clinical records are not stored on the Site.
3. Information Collected Automatically
The Site is hosted on Squarespace. When you visit, certain information is logged automatically by the hosting platform, including your Internet Protocol (IP) address, browser type and version, device type and operating system, referring web address, the pages you view, and the dates and times of your visits.
Squarespace also provides built-in analytics that report aggregate patterns such as page views, traffic sources, and general geographic region. We use this information to understand how the Site is being used and to improve it.
We do not use third-party advertising pixels, retargeting tags, or behavioral advertising technologies on the Site, and we do not use tracking technology to build advertising profiles based on the pages you visit. We do not knowingly permit third parties to collect information about your activity on the Site for their own advertising purposes.
4. Cookies and Similar Technologies
A cookie is a small text file placed on your device by a website. The Site uses cookies set by Squarespace that support basic functionality, security, and aggregate analytics.
Most web browsers allow you to refuse cookies, delete cookies already stored, or alert you when a cookie is being set. Instructions vary by browser and are available in your browser’s help settings. Disabling cookies may affect how parts of the Site function.
Some browsers transmit a "Do Not Track" signal. There is no common industry standard for how such signals should be honored. Because the Site does not engage in cross-site behavioral tracking, no additional response is taken.
5. How We Use Information
Information collected through the Site is used to:
• Respond to inquiries, consultation requests, and other messages you send us
• Schedule, confirm, and manage appointments
• Provide, maintain, and improve the Site and our services
• Maintain the security and proper functioning of the Site
• Comply with legal, professional, and licensure obligations
We do not use information collected through the Site to make automated decisions that produce legal or similarly significant effects.
6. How We Share Information
We do not sell, rent, or trade personal information. We do not share personal information for cross-context behavioral advertising. Information is shared only as described below.
Service providers
We use third-party vendors to operate the practice. Each receives only the information needed to perform its function. These currently include Squarespace (website hosting and analytics), Headway (scheduling, client portal and electronic health records), and our email service provider through Google Workspace. Vendors that create, receive, maintain, or transmit protected health information on our behalf are engaged under HIPAA business associate agreements.
Legal and professional obligations
We may disclose information when required by law, court order, subpoena, or other legal process, or when disclosure is required or permitted by our professional and licensure obligations. These include mandatory reporting of suspected abuse or neglect of a child, vulnerable adult, or elderly person under Mississippi and Alabama law, and disclosure where there is a serious and imminent threat to the health or safety of a person or the public.
Business transfers
If the practice is sold, merged, dissolved, or otherwise transferred, information may be transferred as part of that transaction, subject to applicable law governing the custody and transfer of health records.
7. Third-Party Websites and Platforms
The Site may link to third-party websites, platforms, directories, or resources, including Headway, professional directories, and informational or crisis resources. We do not control those sites and are not responsible for their content, security, or privacy practices. This policy does not apply to them. Review the privacy policy of any third-party site before providing information to it.
8. Data Security and Its Limits
We maintain reasonable administrative, technical, and physical safeguards designed to protect information in our custody against unauthorized access, use, alteration, and disclosure.
No method of transmitting or storing information over the internet is completely secure. Standard email and website contact forms are not encrypted end-to-end and are not a secure channel for sensitive health information. Please limit what you send through these channels to general inquiries and scheduling requests. Once you become a client, secure messaging through the client portal is the appropriate channel for clinical communication.
You are responsible for protecting the confidentiality of any account credentials you use with our scheduling or portal platforms, and for the security of the device and network you use to access them.
9. Notification of a Data Breach
If unauthorized acquisition of unencrypted personal information occurs, we will provide notice as required by Mississippi law, including Miss. Code Ann. Section 75-24-29, and by the Alabama Data Breach Notification Act of 2018 for Alabama residents. Where protected health information is involved, notice will be provided in accordance with the HIPAA Breach Notification Rule.
10. Children’s Privacy
The Site is intended for adults and is not directed to children under the age of thirteen. We do not knowingly collect personal information from children under thirteen through the Site. If you believe a child has provided information through the Site, contact us at contact@foundvoicetherapy.com and we will delete it.
Where clinical services are provided to a minor, the information practices in our Notice of Privacy Practices apply, including the rights of parents and legal guardians under applicable state and federal law.
11. Your Choices and Requests
You may contact us at contact@foundvoicetherapy.com to:
• Ask what information we hold that you submitted through the Site
• Ask us to correct inaccurate information you submitted
• Ask us to delete information you submitted through the Site
• Ask to be removed from any email list you joined
Mississippi has not enacted a comprehensive consumer data privacy statute. We honor these requests as a matter of practice regardless of the state you live in, subject to record retention obligations that may require us to keep certain information.
Your rights regarding your clinical record, including the rights to access, amend, and receive an accounting of disclosures, arise under HIPAA and are described in our Notice of Privacy Practices. Those rights are separate from the requests described in this section.
12. Data Retention
Information submitted through the contact form and by email is retained only as long as needed for the purpose it was provided, for legitimate business purposes, or as required by law. Clinical records are retained according to the retention requirements applicable to licensed clinical social workers in the states where services are provided, and according to the schedule stated in our Notice of Privacy Practices.
13. Changes to This Policy
We may update this policy from time to time. The current version is always posted on the Site with the effective date shown at the top. Material changes will be noted on the Site. Continued use of the Site after a change takes effect indicates acceptance of the revised policy.
