Terms of service.
1. Acceptance of These Terms
These Terms of Service ("Terms") govern your access to and use of the website located at foundvoicetherapy.com (the "Site"), operated by Found Voice Therapy, LLC ("Found Voice Therapy," "we," "us," or "our"). By accessing or using the Site, you agree to these Terms. If you do not agree, do not use the Site.
These Terms govern the Site only. They do not constitute a treatment agreement. If you become a client, a separate informed consent and treatment agreement will govern the clinical relationship, and that agreement controls where it conflicts with these Terms.
2. Who May Use the Site
The Site is intended for adults eighteen years of age or older. If you are under eighteen, please use the Site only with the involvement of a parent or legal guardian, and do not submit personal information through the Site without their consent.
3. The Site Does Not Create a Therapist-Client Relationship
Viewing the Site, reading its content, submitting the contact form, sending an email, or requesting an appointment does not create a therapist-client relationship. No clinical relationship exists until you and Found Voice Therapy have completed an intake, signed an informed consent and treatment agreement, and begun services.
Content on the Site is general information about mental health topics and about the services offered. It is not clinical advice, diagnosis, or treatment, and it should not be used as a substitute for consultation with a qualified professional who knows your circumstances.
4. Services, Licensure, and Geographic Limits
Found Voice Therapy provides outpatient individual psychotherapy. Services are delivered by Raevin Wade, LCSW, a Licensed Clinical Social Worker.
In-person sessions are offered at our office in Hattiesburg, Mississippi. Telehealth sessions are offered to clients physically located in Mississippi and Alabama at the time of the session.
State law requires that a clinician be licensed in the state where the client is physically located when services are delivered. We cannot provide services to anyone located outside Mississippi or Alabama, regardless of where that person resides or holds a mailing address. If you relocate or travel outside these states, tell us before your next appointment so that we can discuss options, which may include pausing services or referring you to a clinician licensed in your location.
The Site describes services that are currently available. Nothing on the Site is a commitment that a particular service, modality, appointment time, or clinician will be available to you.
5. Appointment Requests and Scheduling
Scheduling and the client portal are provided through Headway, a third-party platform. Your use of Headway is governed by Headway’s own terms of service and privacy policy, which you should review. We are not responsible for the availability, accuracy, or security of third-party platforms.
An appointment request submitted through the Site or through Headway is a request, not a confirmed appointment. An appointment is confirmed only when you receive confirmation from us. Availability is limited and is not guaranteed.
We reserve the right to decline a prospective client where clinical appropriateness, scope of competence, licensure, scheduling, or professional ethical obligations require it. Where we decline, we will provide referral information.
6. Fees, Payment, and Cancellation
Found Voice Therapy currently accepts the following insurance: Cigna, Aetna, Carelon Behavioral Health, and Curative while other insurance credentialing is pending. A sliding scale is available; eligibility and terms are discussed directly. Current fees are provided before services begin and are stated in your treatment agreement.
Good Faith Estimate
Under the federal No Surprises Act, clients who are uninsured or who are not using insurance have the right to receive a Good Faith Estimate of expected charges before scheduling services and on request. If you receive a bill that is at least four hundred dollars more than your Good Faith Estimate, you may be able to dispute the bill. Ask for a written estimate any time before scheduling, and keep a copy. More information is available at www.cms.gov/nosurprises.
Cancellation and missed appointments
Appointments cancelled with less than 24 hours of notice, and appointments missed without notice, may be charged at the full session rate. Insurance does not cover missed-appointment fees. The cancellation policy in effect for clients is the one stated in the signed treatment agreement.
7. Electronic Communication
Email and the Site’s contact form are not secure or encrypted channels and should not be used for clinical content, detailed health information, or anything time-sensitive. Use them for general inquiries and scheduling only.
We generally respond to messages within one to two business days during regular business hours. We do not monitor email, the contact form, voicemail, or social media outside of business hours, on weekends, or on holidays.
We do not provide clinical services, advice, or crisis support by email, text message, social media, or through the Site.
8. The Site Is Not for Emergencies
Do not use the Site, email, or the contact form to communicate an emergency. If you are experiencing a mental health emergency, thoughts of harming yourself or another person, or any medical emergency, call 911 or 988, or go to your nearest emergency department. See our Disclaimers page for crisis resources.
9. Intellectual Property
All content on the Site, including text, graphics, logos, the Found Voice Therapy name and mark, page layout, and downloadable materials, is owned by Found Voice Therapy, LLC or its licensors and is protected by copyright, trademark, and other applicable law.
You may view, download, and print Site content for your own personal, non-commercial use. Any other use, including reproduction, republication, distribution, modification, or use in training or developing any automated system, requires our prior written permission.
10. Acceptable Use
You agree not to:
• Use the Site for any unlawful purpose or in violation of these Terms
• Submit false, misleading, or fraudulent information
• Attempt to gain unauthorized access to the Site, its servers, or any related system
• Interfere with or disrupt the Site, including by introducing malicious code
• Use automated means to scrape, harvest, index, or collect content or data from the Site without written permission
• Post or transmit content that is harassing, defamatory, threatening, or that discloses another person’s private information
We may restrict or terminate access to the Site at any time for conduct that violates these Terms.
11. Third-Party Links and Resources
The Site may link to third-party websites, directories, and resources. Links are provided for convenience and do not constitute endorsement. We do not control and are not responsible for third-party content, services, availability, or privacy practices. Your use of any third-party site is at your own risk and subject to that site’s terms.
12. Disclaimer of Warranties
The Site and its content are provided on an "as is" and "as available" basis, without warranties of any kind, express or implied, including any implied warranties of merchantability, fitness for a particular purpose, accuracy, and non-infringement, to the fullest extent permitted by law.
We do not warrant that the Site will be uninterrupted, timely, secure, or error-free, that defects will be corrected, or that the Site or its hosting servers are free of harmful components. Site content is provided for general informational purposes and may not reflect the most current clinical research, professional guidance, or legal requirements.
Nothing in this section limits our professional obligations to clients under applicable licensure law, professional ethics, or a signed treatment agreement.
13. Limitation of Liability
To the fullest extent permitted by law, Found Voice Therapy, LLC and its members, employees, and agents will not be liable for any indirect, incidental, special, consequential, exemplary, or punitive damages, or for any loss of data, goodwill, or anticipated benefit, arising out of or relating to your access to or use of the Site, your inability to access the Site, or your reliance on any Site content, whether based in contract, tort, negligence, strict liability, or any other theory, and whether or not we were advised of the possibility of such damages.
To the fullest extent permitted by law, our total aggregate liability arising out of or relating to the Site will not exceed one hundred dollars ($100.00).
Some jurisdictions do not allow the exclusion or limitation of certain damages. Where that is the case, the limitations above apply to the fullest extent permitted by law. Nothing in this section limits liability for professional negligence in the provision of clinical services, or any other liability that cannot lawfully be limited.
14. Indemnification
You agree to indemnify and hold harmless Found Voice Therapy, LLC and its members, employees, and agents from any claim, demand, loss, liability, or expense, including reasonable attorneys’ fees, arising out of your misuse of the Site, your violation of these Terms, or your violation of the rights of any third party.
15. Governing Law and Venue
These Terms are governed by the laws of the State of Mississippi, without regard to its conflict of laws principles. Any action arising out of or relating to these Terms or the Site shall be brought exclusively in the state or federal courts located in Forrest County, Mississippi, and you consent to the jurisdiction of those courts.
16. Changes to These Terms
We may revise these Terms at any time. The current version is posted on the Site with the effective date shown above. Your continued use of the Site after changes take effect constitutes acceptance of the revised Terms. Review them periodically.
17. Severability and Waiver
If any provision of these Terms is found unenforceable, that provision will be limited or severed to the minimum extent necessary and the remaining provisions will remain in full force. Our failure to enforce any provision is not a waiver of that provision or of any other right.
18. Entire Agreement
These Terms, together with the Privacy Policy and the Disclaimers posted on the Site, constitute the entire agreement between you and Found Voice Therapy, LLC regarding use of the Site, and supersede any prior understanding on that subject. They do not supersede any signed treatment agreement or informed consent document.
